Behavioral Supports

We provide Behavioral Supports to help individuals manage challenging behaviors, build essential life skills, and thrive in their daily environments. Our approach is compassionate, evidence-based, and tailored to each person’s unique needs.

All services are designed to address specific behavioral challenges and support positive, lasting change.

Our Approach

Behavioral Assessment: We conduct a comprehensive evaluation of the individual’s behaviors, triggers, and patterns to understand the root causes and best strategies for support.

Intervention Planning: A personalized Behavior Intervention Plan (BIP) is created with specific goals, strategies, and success measures that promote positive behaviors and reduce harmful or disruptive actions.

Implementation & Support: We work closely with individuals, families, and staff to implement the plan consistently across home and community settings.

Caregiver Training: We equip caregivers and staff with the skills and confidence to support behavior plans through hands-on training and ongoing supervision.

Progress Monitoring: Behavioral goals are regularly reviewed and updated based on data, observations, and team feedback to ensure continued success.

Who provides these supports?

Behavioral services at Hopespring Homes are delivered by qualified professionals such as:

  • Board-Certified Behavior Analysts (BCBAs)
  • Licensed Clinical Social Workers (LCSWs)
  • Licensed Professional Counselors (LPCs)
  • Licensed Psychologists or other credentialed specialists

Each provider is selected based on the specific needs outlined in the individual’s Service Plan.

Community-Based Supports

We offer Community-Based Supports to help individuals lead more independent, meaningful, and connected lives. These one-on-one services are designed to help individuals achieve their personal goals both inside and outside the home.Community-Based Supports provide direct assistance to individuals with or without a caregiver present to help them:

  • Build independence
  • Strengthen life skills
  • Increase social participation
  • Improve family functioning
  • Access their community safely and confidently

All services are personalized and aligned with the individual’s Service Plan.

What’s Included?

Supports are delivered one-on-one and may take place in the home, in the community, or at other appropriate locations. Here are some examples,

– Attending a class, recreational event, or community program

– Ordering food at a restaurant or making purchases at a store

– Learning to navigate public transportation (travel training)

– Engaging in community-based hobbies and group activities.

Life Skills & Independence

– Cooking simple meals and basic food safety

– Doing laundry, household chores, and personal care routines

– Managing money or understanding budgeting basics

– Practicing good hygiene and grooming

One-on-One Support

– Individual tutoring or learning support

– Social skills development

– Building confidence through guided practice in everyday situations

Support on a Job Site

– Assistance with self-care needs while at work

– Staying focused on job tasks

– Practicing appropriate social interactions

– Lunch and break-time support

Who Delivers These Services?

Our support team is trained to provide compassionate, professional care with a focus on personal growth and inclusion. All staff members meet or exceed qualifications required by the NJ Division of Developmental Disabilities (DDD).

Note: Community-Based Supports on the job site are not a substitute for Supported Employment services. Any training directly related to learning job duties must be provided by a qualified Supported Employment provider.

Individual Support Services

We provide Individual Supports tailored to each person’s needs, goals, and lifestyle. Whether in the home or out in the community, with or without a caregiver present, our team works side-by-side with individuals to build the skills and confidence needed to live fuller, more independent lives.

  • Increasing independence and self-care
  • Improving social skills and community involvement
  • Enhancing daily routines and overall quality of life
  • Supporting family caregivers by reinforcing consistency and progress at home

We meet each person where they are — emotionally, developmentally, and physically — and help guide them toward what’s next.

What Can Be Included?

Supports are flexible and customized to match the outcomes in the individual’s DDD Service Plan. Examples include:

– Support with activities of daily living (bathing, grooming, dressing)

– Assistance with basic home management (laundry, meal prep, organization)

– Support during community-based activities (shopping, dining, attending events)

– Coaching around etiquette, communication, and social interaction

– Skill-building for travel training and navigating public transportation

– Supervision and safety awareness during daily tasks or outings

These services can occur in-home or out in the community, depending on the individual’s preferences and goals.

Support From Trusted People

Hopespring supports Self-Directed Employees (SDEs) as part of our flexible service model. Family members can provide Individual Supports as long as:

– They meet the same qualifications, training, and background standards as non-family providers

– The arrangement is consistent with the individual’s DDD-approved Service Plan

This flexibility allows care to be both personalized and professional, ensuring trust and continuity.

Occupational Therapy

At Hopespring Homes LLC, our Occupational Therapy (OT) services are designed to help individuals gain or regain the skills needed to participate in daily life with greater confidence and independence.

  • Helps individuals of all ages build or restore the physical, cognitive, and sensory abilities
  • Personalized and goal-driven
  • Habilitative – building new skills the individual has not yet developed
  • Rehabilitative – restoring skills that may have been lost due to injury or health conditions

Our therapists focus on practical improvements that make a real difference in the individual’s quality of life at home, in school, at work, and in the community.

Our licensed occupational therapists work with individuals one-on-one or in small groups to target goals such as:

– Fine motor skills (e.g., writing, buttoning, using utensils)

– Self-care activities (e.g., dressing, grooming, bathing)

– Sensory processing challenges

– Hand-eye coordination

– Daily living skills and routines

– Strength, balance, and body awareness

– Adaptation of tasks or environments for better access and comfort

Individual or Group Sessions Available

Occupational Therapy sessions can be delivered in two formats:

– Individual Therapy: One-on-one support tailored to personal goals and needs

– Group Therapy: Small group sessions (up to 5 participants) led by one therapist, focused on shared skills such as teamwork, social communication, or classroom readiness

Note: Group sessions are always led by a licensed occupational therapist and follow strict quality and participant ratio guidelines.

What Services Are Included?

How is This Service Funded?

Occupational Therapy services follow the standards and provider qualifications outlined in the New Jersey State Plan. If an individual has already used all of their OT benefits under the State Plan, additional support may still be available through their DDD-funded services.

We’ll work with families and support coordinators to ensure access to the care each individual needs.

Respite Care Services

At HopeSpring Homes LLC, we understand the importance of rest and renewal for caregivers. Our Respite Care Services provide safe, compassionate, and temporary care for individuals, allowing family members or regular caregivers the time they need to rest, recharge, or attend to other responsibilities.

  • Short-term support service for daily living.
  • Temporary relief to family members, guardians, or caregivers.
  • Few hours or several days, depending on the need.
  • A trained team steps in to deliver care during the caregiver’s absence.
  • Ensure the loved one remains safe, supported, and engaged in a caring environment.

All services are personalized and aligned with the individual’s Service Plan.

Flexible Duration Options

We offer respite care in various formats to meet the unique needs of each family:

  • Partial-hour support
  • Hourly sessions
  • Daytime care (without overnight stay)
  • Overnight care (daily or multi-day stays)

 Where is Respite Care Provided?

Respite services can take place in several approved and comfortable environments:

  • The participant’s home
  • A DHS-licensed group home
  • An approved community-based setting
  • A state-approved alternative location, such as a hotel, if other options are not available

All settings are designed to provide a safe, supportive, and supervised environment tailored to the participant’s needs and comfort.

Safe, Reliable, Compassionate

Our respite staff are trained professionals who prioritize the safety, dignity, and individual needs of each person we serve. We follow all standards and guidelines established by the New Jersey Division of Developmental Disabilities (DDD) and the Department of Human Services (DHS).

Transportation Services

At Hopespring Homes LLC, we provide Transportation Services designed to help individuals travel safely to and from the essential services, resources, and community activities outlined in their personalized Service Plan. Our goal is to support independence, inclusion, and full participation in everyday life.

  • Attend community activities and programs
  • Access employment or volunteer opportunities
  • Participate in day services, therapy, or appointments
  • Connect with friends and social events

We make sure individuals reach their destinations safely, on time, and with the dignity they deserve. Please note: This service does not replace Medicaid or State Plan medical transportation services.

Our Transportation Services are:

  • Flexible: Provided in addition to any State-mandated transportation support
  • Convenient: Designed to help individuals access services listed in their Service Plan
  • Cost-Conscious: Reimbursable for trips up to 150 miles one-way

Safety First

All vehicles and drivers used by Hopespring Homes LLC meet or exceed the New Jersey Motor Vehicle Commission safety standards. We are committed to:

  • Operating vehicles that are licensed, insured, and regularly maintained
  • Providing wheelchair-accessible vehicles with lifts and securement systems
  • Ensuring staff are trained and certified to provide safe, respectful transportation

What's Included?

Each vehicle is equipped with:

  • First Aid Kit
  • 10:BC Dry Chemical Fire Extinguisher
  • At least 3 portable red reflector warning devices
  • Snow tires or chains when required by weather

Who Can Use This Service?

Transportation Services are available to individuals:

  • Enrolled in the DDD program
  • Whose Service Plan identifies a transportation need
  • Who do not already have access through family, friends, or free community options

Our team works with families and support coordinators to ensure proper authorization and scheduling.

Transportation is not available for services that already include built-in transportation, such as Day Habilitation or Daily Individual Supports within their assigned region.
