    Hopespring Homes LLC provides safe and reliable transportation to help individuals access essential services, community activities, employment, and social events as outlined in their personalized Service Plan. Our services are flexible, cost-conscious (reimbursable for trips up to 150 miles one-way), and designed to complement—not replace—existing Medicaid or state-funded transportation.

    Who is eligible for Transportation Services?

    Transportation Services are available to individuals: Enrolled in the NJ Division of Developmental Disabilities (DDD) program With a documented transportation need in their Service Plan Who do not have access to transportation through family, friends, or free community options We prioritize safety, ensuring all vehicles meet NJ Motor Vehicle Commission standards and are wheelchair-accessible.

    What is Respite Care, and how can it help caregivers?

    Respite Care provides short-term relief for family members or primary caregivers, offering temporary support in a safe, supervised setting. It helps prevent caregiver burnout by allowing time for rest, errands, or emergencies. Services can be provided in the individual’s home, a licensed group home, or an approved community setting for a few hours, days, or overnight stays.

    How does Occupational Therapy (OT) at Hopespring Homes work?

    Our OT services help individuals develop or regain skills needed for daily living, such as fine motor skills, self-care, sensory processing, and adaptive strategies. Services can be habilitative (building new skills) or rehabilitative (recovering lost abilities) and are delivered one-on-one or in small groups. Funding may come through NJ State Plan benefits or DDD services if State Plan benefits are exhausted.

    What are Behavioral Supports, and who provides them?

    Behavioral Supports include assessments, intervention plans, counseling, and caregiver training to address challenging behaviors and promote positive change. Services are delivered by qualified professionals like Board-Certified Behavior Analysts (BCBAs), Licensed Clinical Social Workers (LCSWs), or Psychologists. Each individual receives a customized Behavior Support Plan based on their needs.

    What’s the difference between Individual Supports and Community-Based Supports?

    Individual Supports focus on one-on-one assistance with daily living skills (e.g., grooming, meal prep) at home or in the community. Community-Based Supports emphasize social participation and independence (e.g., navigating public transit, attending events). Both services align with the individual’s DDD Service Plan and can include family members as qualified providers.
